Where public City materials place common employee tax records and what to do if a form is missing or needs correction.

The BPS Office of Human Resources lists W-4, W-2 and 1095 forms under ESS employee financial information. Start from the official Access Boston portal and use the ESS records available to your account. If your department directs you through Beacon, follow that current employee route. A form location is not tax advice: this guide does not choose withholding, interpret a tax return, or determine which form applies to an individual.

First identify which document you need. A W-4 is a withholding election; a W-2 reports wage and withholding information for a tax year; a 1095 form relates to health coverage information. These records serve different purposes. The City’s ESS page lists the forms but does not provide tax preparation guidance. Use the form label and year shown in the official system before downloading or contacting support.

Find the form from the employee record system

Open ESS using the assigned City route and look under the financial or tax-document function that your current screen offers. The public ESS page names these document types but may not show the exact current navigation labels or every employee group’s view. Use the in-account instructions if the menu changes. Download a form only to a device you control. Check the document year and employee identity before saving or printing. Avoid leaving a copy in a shared download folder or attaching it to a public support request. This publisher does not accept tax forms, Social Security numbers or screenshots; contact the City through its verified secure employee channel.

This table is a plain-language orientation, not a complete tax description. The IRS, a tax professional, or current City instructions may explain how a specific form applies. Do not change a W-4 merely to resolve an unrelated paycheck discrepancy; withholding choices can affect future tax treatment and should be made with appropriate guidance.

If the form is missing or a detail looks wrong

Confirm that the selected year and account are correct, then check current department announcements or the official employee instructions for availability dates. The public ESS page identifies where forms may be accessed but does not promise a release date for every document. If the record remains missing, contact the department payroll, HR or benefits team that owns it and state the form type and year. Use the secure channel it designates.

If you see an incorrect name or address, compare it with the personal information on file and ask HR which source record controls the tax form. A change entered in ESS may not automatically update a previously issued document. Do not assume that an updated profile rewrites an already finalized tax record; ask the responsible office how corrections are handled.

Keep forms private

  • Use only the official employee portal or a verified City instruction.
  • Store downloaded forms in a private location and remove copies from shared devices when finished.
  • Do not send a tax form or Social Security number to this editorial publication.
  • Share a document with the City only through the secure method its verified office specifies.
  • Ask a qualified tax resource about tax treatment rather than relying on a portal guide.

A document can contain sensitive financial and identity information. The fact that the City offers an online download does not mean a form should be emailed casually. If a message unexpectedly asks you to upload tax documents to a new link, verify the request through a known official City contact before acting.

Record location and professional advice are separate

ESS answers where a listed employee record may be available. It does not decide a tax filing position, estimate a refund, resolve a personal tax notice or guarantee that every employee sees the same form. For a missing or incorrect employee document, use the designated City payroll, HR or benefits route; for tax interpretation, use a qualified tax authority or professional.
